For the proper course of treatment, it is essential that PsyPerspectief creates and maintains a medical record. This record contains necessary notes regarding your health condition, as well as information about conducted examinations and treatments. Additionally, the record includes data crucial to your treatment that PsyPerspectief, with your explicit consent, may have obtained from other medical professionals, hospitals, or the company doctor.

Data in your record may be shared with other healthcare providers, provided you do not explicitly object. For instance, in case of referral, data may be shared with medical specialists, general practitioners, or pharmacists. A limited set of data from your record is used for administrative purposes and audit purposes. Moreover, data from your record may be shared with third parties if legal regulations or contractual obligations require it, such as with health insurers.
